As a Home Health Nurse, you'll enjoy the autonomy to schedule patient visits according to your availability while delivering 1:1 care. Collaborate with your team on treatment plans, think creatively with available resources, and educate patients and their families on adapting care to meet evolving needs.

Job Summary:

In this role, you'll coordinate, facilitate, and perform interdisciplinary plans of care through assessment and evaluation of patient conditions. You'll travel to patient homes, Community-Based Residential Facilities (CBRF), assisted living locations, and hospitals to meet patient and family needs.

Job Responsibilities:

• Assess and coordinate patient discharge planning needs with the healthcare team.
• Perform point-of-care testing as per policies and procedures.
• Collaborate with physicians regarding care plans and patient status updates.
• Communicate with multi-disciplinary team members and outside agencies to achieve expected outcomes.
• Complete admission paperwork and verify insurance information.
• Provide skilled nursing care, including vital checks, wound care, IV care, and more.
• Document visit notes and ensure timely, accurate records.
• Develop comprehensive care plans in collaboration with patients and families.
• Maintain a constant state of alertness and safety.
• Perform other duties as assigned.

Requirements:

• Education: Graduate of an accredited school of nursing or equivalent.
• Experience: No prior experience required.
• Additional Requirements: Proof of motor vehicle liability insurance upon hire.
• Licenses/Certifications:
  • RN or LPN licensure (from the state where work will be performed)
  • BLS (AHA)
  • State-issued Driver's License
  • Motor Vehicle Report

Physical Requirements:

• Constant use of speech for communication.
• Frequent standing, walking, and lifting/carrying objects weighing up to 25 lbs.
• Frequent use of vision and hearing for patient assessment.
• Occasional lifting/moving of patients and objects weighing up to 50 lbs.
