Summary: Family Model Provider: We are looking for caring individuals who share our dedication to providing a supportive environment for persons with intellectual and /or Developmental Disabilities (I/DD) in their home. We ask that you bring a desire to serve others and share your home, and RHA will provide the training and support to ensure your success. As a Family Model Provider, you will be asked to provide support for individuals with disabilities in a home setting. You will be asked to provide the supports identified below based on the needs of the individual in your care. Position Requirements: HS Diploma/GED preferred, 18 years old or older, Documentation of home ownership: current mortgage statement or renter’s lease, Proof of homeowner’s or renter’s insurance or personal property insurance, Valid Driver’s License; Reliable transportation with valid vehicle insurance, Ability to pass criminal background and Registry checks as well as a Drug Screen, TB Screenings for all members of household, Willing to be a self-employed worker. Responsibilities: Seek to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Support the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working. Supports are provided according to the person’s abilities and interests while ensuring privacy and dignity. Prepare, teach and assist the person in food preparation, and cleaning of the home. Honor the choices, preferences and culture of the person you support. Assist in providing opportunities for social, emotional and spiritual needs to be met. Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities. Ensure that the home environment is safe and in good condition. Implement services and supports to address goals identified through the Person Centered Plan. This includes medical, psychiatric, recreational, spiritual, and emotional needs. Assist the person supported by attending team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals. Follow state guidelines specific to Family Model Services. RHA will assist you in the following capacities: Provide support through a multidisciplinary team for crisis assistance and training as needed as you begin your journey as a Family Model Provider. Training includes RHA’s proprietary ProAct training, which focuses on supporting resiliency, emotional stability and problem solving. Supervision will be provided to assist you in adjusting to your new role. The frequency and amount of supervision will be determined in a collaborative relationship with your RHA supervisor. Pre-employment screening includes criminal background check, registry checks, drug testing, education verification, proof of employment history or references, and if driving is required, proof of driver's license, insurance, and vehicle.
